Making an Appointment

Appointments are reservations for professional dental care. We strive to see patients promptly and to provide a home like family environment.  To schedule your reserved appointment or for questions e- mail us at smile@gatordental.com or phone us at (352) 481-2741.  Internet automated appointment scheduling is not provided as we need to discuss the details of your needs so we can match an appointment time with the personnel, treatment room and time needed to offer you the best in care.

We have published all our new patient forms in a convenient online package for you. Click on the link and the package will open using Adobe Reader. Please print the forms (use the printer icon at the top of Adobe Reader) and fill them out completely. 

When done either Fax to (352) 481-5341 or mail to Gator Dental Associates PA, PO Box 10, Hawthorne, FL 32640.  This will better help us to prepare for your first visit, allowing us to focus on your care when you arrive!

Existing patients needing to complete newly required government HIPAA acknowledgement of privacy practices should click, open, print and complete the following.  Fax the signature page to us at 352-481-5341.
