First Visit

Appointments

We know you have many choices for dental care in  so we have made requesting an appointment a simple process via our Web site or via text to 352-481-2741.

We want your first visit to be more about getting to know you and less about paperwork, so we highly recommend filling out your New Patient Forms prior to your appointment.   Please provide us with a cell phone number, or email, and we can provide a link to fill out paperwork prior to your visit to help save you time at our office.

Please plan to arrive 10-15 minutes prior to your appointment time in case a delay should occur, such as traffic. Your appointment is reserved especially for you. If you arrive late, your appointment may need to be cancelled or shortened in order to accommodate other patients who have appointments after you. Filling out paperwork when you arrive at the office can decrease the amount of scheduled time we have to do your treatment as well. If possible, please fill out the digital forms we send prior to your appointment with us.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable.
  • A full list of medications you are presently taking.
  • If you have dental insurance, bring the necessary completed forms. Or, provide to the front desk ahead of time. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, they can be taken at our facility.

If, for any reason, you cannot keep a scheduled appointment, or will be delayed, please contact us at least 24 hours in advance, when possible.  Otherwise a broken appointment fee of $30 may be incurred. We also understand that last minute emergencies happen and will work with you to reschedule your appointment.
